Episode 43 starts with a troubling case of a woman from Detroit who suffered acute hemorrhagic necrotizing encephalopathy - or a bleeding brain - apparently related to Covid and the cytokine storm problem. We look at a study from Los Angeles that showed only 5% of the suspected influenza patients had Covid, meaning both coronavirus spread is in the community there but also still likely has many more people to reach foreshadowing a longer interval. That's bad news for doctors being silenced and fired for talking about a lack of PPE leading to situations like Boston and Spain where mass staff infection is becoming common. In NY, the situation is seeing mass graves being predug by prisoners. The fortieth episode looks at the second part of the health equation which is to delve into why the economic problem is so bad. After a quick review of drug shortages, an exploration of a different pathway by which the coronavirus is harming people, and a glance into Russia getting sick and Italy having dead whose numbers are rising sharply in one town without an attributed reason, we look at the pending policy bill. A pork-laden monstrosity that is conceived reactively as just another response in an unending line of new debts, it fails to solve the basic question of how to keep people whole and stop people from moving for a time sufficient to stamp out Covid. The rush to satisfy debt versus public health means a bill not so concerned with ventilators, PPE, and medical support as it is inadequate loans, heavy bureaucracies, and promising a bigger problem later for many people. We see 3.3 million Americans unemployed this week as the first harbinger of many more to come. Inability to pay rent, then to pay mortgages, and then to pay lenders comes forward as small businesses shutter. How does agriculture hold up especially with a severely at risk population for both spread and contamination in close quarters outside legal status still out there? Absent these answers, we see truckers avoiding NYC, people in northern Michigan telling city folks to stay home, and Texas now quarantining people coming from hot spots being quarantined or heavily fined and incarcerated. All this in a battle between finance, debt, and control, with economy, assets, and scarcity on the other side as people make rational health decisions toward safety, something our government seems to struggle on all sides to understand.

Episode 39 is the first part of an exploration about the dangers America faces and what we realistically should respect from this virus. After a quick update in which it appears reinfection is less likely as good news but mutation more aggressive as bad news, we go into the action in New York's hospitals where life and death scenes are playing out right now as supply is being overwhelmed. The stark relief comes into focus with two pieces by the New England Journal of Medicine that consider rationing of care, and ventilator protocols as what some will call triage and others will brand death panels because of our inaction. We see morgue capacity now in question, officers calling off sick, and doctor breaking down as garbage bags are being used to supplement PPE. This crisis may go on for many months, be a season event, and we look starkly at the potential of event that without much better action that seems improbable from not just policy makers, but our businesses and our people, where Americans are chancing a death toll into the millions by summer's end. Medical imports are lagging, chloroquine is running out, and the search for PPE is becoming critical as the CDC is telling nurses to wear bandanas if all else fails. We look into the quarantine letters about essential businesses and see that as an effort to keep people out, but at what expense to the virus fighting effort as many people in Seattle choose to ignore stay at home orders. As DC fights over a pork laden bill, we look at the debt balloon, the question of inflation, and listen to the Fed warn nearly 1/3 of people may lose their job and half our GDP may also be lost. We look at three ways to deal with that: Resetting the debt, sending in the military, or trying to limp through, admitting there is no perfect solution to a pandemic running roughshod.

We also found a study from China which reports that Tocilizumab is a drug which helped patients in severe distress recover lymphocytes and improve blood oxygen saturation, a chance to reduce the ventilator shortage. The government is working with social media to gain information even as suppression efforts continue, including more analysis of the leaked US Health and Human Services report where they anticipate supply shortages for the next 18 months or longer. The Trump Administration appears to be contemplated a ban on domestic air travel, shutting down the stock market, and asking people to stay in place. However, the existing bill before Congress slowly working forward is not adequate to meet the needs of many people to accomplish such a quarantine meaning absent much improved medical intervention, the spread will likely hasten. Reports from California, Massachusetts, and Pennsylvania all show the acceleration of the coronavirus and slowing of the economy. While government thinks small, people come up big as 3D printers help keep people safe and alive in both New York and Italy, showing the value of open source community based action.

For our thirty-third episode, we start by noting that Facebook is now blocking legitimate stories which they attribute to a "bug" as the WHO and partners invest more energy into controlling information than fighting the outbreak. We share a new study that suggests children can still be at risk, though thankfully less than adults, as we also report from the Dutch that many young seemingly healthy people are in severe distress from Covid. The hope is people will change their behaviors as New York is perched on the verge of a major shutdown that we see happening now in Switzerland. We look at the problems of renters, unemployed, and those needing resources as a bipartisan consensus forms around debt suspension or forgiveness and the direct payment of income to all adult Americans to get through this crisis so people won't have to go out. As that lurches forward, we see Texas getting prepared with better decisions about public events, but shortfalls in supply. Louisiana is also coming under the gun post Mardi Gras, and while Florida has begun shutting down some bars and restaurants, partially, the Sunshine State continues to lag. Lastly, a new drug for fighting flu in Japan is having some positive impacts so we talk about favipiravir (Avigan) as another tool in the arsenal.

For the thirtieth episode, we take stock of what we have to expect as the first heavy blast of the coronavirus outbreak is hitting the West beyond ravaged Italy. A story from Hong Kong talks about lost lung capacity from survivors who at least, in the short term, seem likely not to be immediately reinfected based on best research. The war against Covid is furious in Italy, and we share a personal narrative of what it was like seeing the world a person knew disappear in two weeks to the novel reality of the coronavirus contagion. Modeling explains why we cannot simply flatten the curve, but must change how we live for a time - potentially quite a while - to overcome this by containing ourselves. Against that backdrop, we see policy leaders from the President onward still reacting and taking minor steps against a problem more major than they seem able to comprehend. The DOD is preparing to shut down travel for sixty days for military personnel and families. We should probably be thinking no less as this weekend may be the last chance for travel and resupply for some time. Episode 28 goes for a deep dive into science and policy about the coronavirus outbreak. We start with a new study which informs us Covid-19 can survive as an aerosol for up to three hours contradicting the earlier claim by the CDC that masks were unnecessary, a major problem as we are desperately lacking supplies despite efforts to ramp up production. We present a study that argues for immune suppression capacity of Covid-19 with reduced white blood cell count, but which offers also a useful diagnostic to predict case discovery and prognosis. Another study informs that people can have influenza and coronavirus simultaneously, leading to the testing errors, and perhaps increased risk. We look closely at how chloroquine offers the best hope for immediate aid, especially early in the infection, as it serves to basically jam the lock of the ACE-2 receptor which Covid-19 has most commonly been using. This treatment already pioneered in China and elsewhere is further amplified by using Zinc in conjunction to make the cell even less vulnerable to the virus in a method already being applied by Korea. Solutions exist to mitigate the crisis, but the policy continues to lag in the United States. While there is a welcome departure toward more serious consideration, the reality is both sides are still seeking to mitigate the crisis in a reactive mindset driven more by economics and ideology than understanding this is a war where a week's delay will mean fatal consequences for many people. It's a sobering Episode 19 where we start by looking at rapid worldwide progression of the Covid-19 pandemic. We start by evaluating risk factors to have a better understand of this disease, noting the elevated cancer, diabetes, and obesity numbers, as well as having a larger elderly population, suggest our vulnerable population percentage may be higher than China. Two new articles are covered in exhaustive detail cataloging both that Covid may behave like AIDS in actively suppressing the immune system, and detailing how that function could be causing people to literally forget to breathe due to neurological damage that appears likely to be enduring as Covid has been demonstrated to breach the blood-brain barrier. The CDC continues its string of bungled quarantines and releasing sick patients to the general public as we compliment the FDA for higher competence in working to get test kits out there, and we look at a promising serological test which can potentially identify those previously, or perhaps in remission, with their illnes that is being developed in Singapore. We look at evidence spread happened sooner, wider, and more often than predicted, offering our own modeling to better understand the disease. Episode Six of Coronavirus Central looks much more deeply into the ACE-II pathway by which Covid infects humans and what implications result. We start by examining the nature of the Angiotensin Converting Enzyme, revealing it is an essential component of all humans with heavy clusters in the lower respiratory system, and being used to regulate blood pressure. As we review news and studies based both on Covid and SARS which uses this same mechanism, we learn of the threat not just from infection, but from reinfection where the immune system may begin attacking our own cardiovascular function when it tries to fight off this virus, in a cytokine storm similar to what happened with the Spanish Flu. Coronavirus Central - Episode 5 - February 14, 2020
In Episode 5, we review a study that suggests a far higher case count in Wuhan than previously reported by using statistics to determine both the number of asymptomatic carriers as well as to offer analysis of how effective the quarantine efforts are proving. Coronavirus Central - Episode 4 - February 13, 2020
In Episode Four, we review the change in criteria by the Chinese government to include clinically diagnosed cases which saw a jump of 14,480 new cases and 242 new deaths reported on February 12th. These new criteria bring the numbers closer to what mathematical modeling predicts as we share two studies that suggest the transmission rate should be between 5 and 6 with an incubation period as long as 24 days observed. This means the 14 day quarantines are ineffective, as the spread of cases into the Philippines, Singapore, Thailand, Vietnam, and others makes clear, from which global air and trade continues unabated.
