Dr. Sarah Taber began crop consulting in 2006. Her work since then has focused on regulatory compliance (including food safety), IPM, crop care, and making work flows as efficient as possible in farms and facilities. Through ongoing work as a food safety auditor, Dr. Taber has developed an expert “inspector’s eye” for  potential issues in a facility or blueprint before they cause problems during operation.

Dr. Taber has also enjoyed serving as Director of Food Safety for the Aquaponics Association since 2013. This role has led to some extraordinary opportunities– being on the front line of an emerging market, cutting-edge research, and policy development for the aquaponics industry.

When not working with clients or developing aquaponics-friendly agricultural policy, Sarah Taber enjoys spending time with her family and contributing to the inevitable heat death of the universe at the gym.

PAPod 175 - Just Culture: The Movie - Sidney Dekker's Interview on the Documentary

A just culture is a culture of trust, learning and accountability. In the wake of an incident, a restorative just culture asks: ‘who are hurt, what do they need, and whose obligation is it to meet that need?’ It doesn’t dwell on questions of rules and violations and consequences. Instead, it gathers those affected by an incident and collaborates on collectively addressing the harms and needs created by it, in a way that is respectful to all parties. It holds people accountable by looking forward to what must be done to repair, to heal and to prevent.

You know when you go to a director's screening of a movie that the director comes out on stage after the audience has seen the film...I don't know if you have ever been invited to one of those fancy showings - I can't remember ever getting an invitation to one...that is what this episode is like...you chance to talk to the producer of a docemantary about safety and just culture.

This film documents the amazing transformation in one organization — Mersey Care, an NHS mental health trust in the UK. Only a few years ago, blame was common and trust was scarce. Dismissals were frequent: caregivers were suspended without a clear idea of what they might have done wrong. Mersey Care’s journey toward a just and learning culture has repaired and reinvigorated relationships between staff, leaders and service users. It has enhanced people’s engagement, joint ownership and sense of responsibility. It has taken the organization to a place where hurt doesn’t get met with more hurt, but with healing.

I met Jeremy Cesena the other day at PG&E - he is a friend of the pod and a big thinker...and a great guy.   He decided that he could use the blue line and black line cartoon as a way to code events - like a timeline for understanding and learning from an event.

PAPod 170 - Data is the Fuel, Collaboration is the Engine! Meet The Honorable Chris Hart

Chris Hart was appointed Chairman of the National Transportation Safety Board on March 16, 2015. He became a Member of the Board in August 2009, and was subsequently designated Vice Chairman by President Barack Obama. Hart was nominated for a second term as Board Member in August 2013, and his nomination for a third term as Vice Chairman was confirmed by the Senate in October 2013. He served as Acting Chairman from April 26, 2014, until he was appointed as Chairman.

From 1973 until 1990, Chairman Hart held a series of legal positions, mostly in the private sector. He joined the Board for the first time in 1990 and served until 1993. From 1993 until 1995, he was Deputy Administrator of the National Highway Traffic Safety Administration, then went on to serve as Federal Aviation Administration (FAA) Assistant Administrator for System Safety and FAA Deputy Director for Air Traffic Safety Oversight before returning to the Board in 2009.

Chairman Hart holds a law degree from Harvard University and master’s and bachelor’s degrees in aerospace engineering from Princeton University. He is a member of the District of Columbia Bar and the Lawyer-Pilots Bar Association, and is a licensed pilot with commercial, multi engine, and instrument ratings.

Chairman Hart’s family has a tradition of accomplishment in transportation. In 1926, his great uncle, James Herman Banning, was the first African-American to receive a pilot’s license issued by the US government.

Hart’s 2-year appointment as Chairman expired on March 15, 2017. His 5-year term as Member ended December 31, 2017.

Erik Hollnagel is presently Professor at the University of Southern Denmark and Senior Professor at Jönköping Academy, Sweden. Erik is also Adjunct Professor at Central Queensland University (Australia), Visiting Professorial Fellow, Faculty of Medicine and Health Sciences at Macquarie University (Australia), Visiting Fellow of the Institute for Advanced Study of the Technische Universitat München (Germany), and Professor Emeritus at the Department of Computer and Information Science (IDA) at Linköping University (LIU), Sweden. Prior to that he has held the following positions:

Dr. Scott A. Shappell is a Department Chair and Professor of Human Factors at Embry-Riddle Aeronautical University. Before joining the faculty at Embry-Riddle, Dr. Shappell was a professor of Industrial Engineering at Clemson University. Prior to being on the faculty at Clemson, he was the Human Factors Research Branch Manager at the Civil Aerospace Medical Institute.

In addition, Dr. Shappell served over 16 years in the U.S. Navy as an Aerospace Experimental Psychologist. He has published and presented well over 200 papers, books, and presentations in the fields of accident investigation, system safety, spatial disorientation, sustained operations and fatigue. Dr. Shappell received a B.S. in psychology (1983) from Wright State University graduating Summa Cum Laude with honors in psychology and a Ph.D. in Neuroscience from the University of Texas Medical Branch in 1990.

I really struggled to make this podcast.  Not because I didn't have stuff to say (I always seem to have stuff to say) but because I was afraid I would make people angry.

I think that is a stupid reason to not do a podcast.

I hope this makes people angry - although I softened this message a lot before I posted this message.  

We need to let go of our need to blame somebody for not predicting these events and start to really talk about a system prevention to this very unique problem we have in the United States.

Our need to blame is getting in the way of our ability to improve.

PAPod 158 - The Hardest Conversation I Have Ever Arranged - Here is the Amazing Bill Hoyle!

Bill has more than 30 years experience in process safety and the investigation of major incidents. In the 1990s, he was the Wharton School of Business roundtable on major accident prevention as well as the EPA Experts’ Delegation that studied European safety practices while touring Germany. He was a co-founder of U.S. Chemical Safety Board (CSB) operations in 1998. Hoyle managed the CSB investigation of the 2005 fire and explosions at BP Texas City refinery that killed 15 and injured 180. He was senior investigator for the CSB’s investigation of the Deepwater Horizon disaster as well as the West, Texas fertilizer explosion that killed 15 responders. He was a member of the National Academy of Sciences panel that studied safety culture in the offshore energy industry. Hoyle served as keynote speaker at numerous safety conferences throughout Europe and Australia.

He was expert adviser to CalOSHA and CalEPA on revisions to their regulations for oil refineries. Hoyle provided extensive training for California state and local government refinery safety inspectors.

He is a peer reviewer of the California Council on Science and Technology report on the safety of underground natural gas storage facilities. Mr. Hoyle is an alternate member of the ANSI Z-10 Committee on Occupational Health and Safety Management Systems.

This project was crowd-funded and is a beautiful and effective story of what a journey to change looks like - for organzations that want to be even safer, even more robust, even more effecite.  These are organizations that want to experience safety differently.

Sidney Dekker and his partners in the documentary don't tell or lecture the audience - they simply allow three stories to emerge into one theme.   Hope in each other.

If you think workers have choice, the illusion of free-will, then you have to also think that the worker fully understands the choice about to be made.

Workers get the blunt of pretty much all the pre-job briefing discussions, training, responsibility, and accountabilty.  Workers do all the pre-job work - all the time.

But here is a question:  When is the last time a manager had to do a pre-job brief?  Would they be good at briefs like this?  Would these even work for the work they do?

It is NOT that we are ascribing pre-job briefs for managers (may not be a bad idea), but more that Marc Yeston talks about how managers should THINK about their responability for making pre-job briefs successful in thier organizations.   It is a good thing for us to think about.

Let's face it....new is way sexier than old.  We love new stuff.  We love new construction, new buildings, new equipment, new computers.  We love all this stuff - sometimes - at the expense of maintaining the old.   That topic is what this podcast tries to discuss.

Maintenance is very important.  We have to value the men and women who do the heavy lifting everyday - keeping our facilities up and running.  

We must combat the "bias towards the new" in our thinking as well.

This weeks podcast is an interview done by the world famous Bob Edwards...and I do mean world famous.  Bob had a breakfast interview with Gary Tang, the owner of one of the largest EHS consultancies in China.  Gary was formally as Safety Person for GE...he saw an opportunity to get in to the EHS China market and took it.   He is a leading thinker in a huge market and he is doing well.  

Bob talked to Gary and his associates about the New View and Safety Differently.  It is a great conversation.  This is a world to watch.  Safety is only just beginning in this working world.

Safety Moment - On Being Right

Being right is not nealy as valuable as being curious.  Being right simply re-inforces the fact that you were right.  You learn nothing and therefore you know nothing new.  It is far better to be curious than right.  When you are curious you are constantly learning.  Knowing more makes you smarter!

I hope you noticed that I did not use the word wrong.  I don't care much about being wrong either...I want you to know the power is in not knowing, the power is not so much in being right or being wrong.

This safety moment is maybe the most important advice we can hear - and we don't hear it enough.

There is an old joke - "How do you eat an elephant?  One bite at a time!"  In many ways that old joke - I never said funny joke - is a good lesson for us.  Change happens one bite at a time - not all at once.

There is power in incrementalism.  Small changes matter!  We change the world one conversation at a time.   Don't forget that!

Cause is a construct - not a real thing.   

Sorry you guys.  I know that makes some of you crazy and you want to argue with me a lot about why that idea is wrong.  The problem is that the idea is not wrong - Cause is a construct that is attributed to an event - it is never real and never singular.
